Free Mailer Packet

If you would like to receive a free prepaid mailer for your next
repair please fill out the Free Mailer Packet form.

Mailing Instructions

1)   Upon receipt of mailer, just wrap STERILIZED handpiece(s) in bubble wrap.

2)   Place handpiece in a sturdy box, and add additional packing material; i.e. newspaper, foam peanuts to keep contents from shifting.

3)    Include a completed Service Repair Form

4)    Tape box shut with packaging tape only.

5)    Tape free mailer to outside of box.  Then just give your prepaid box to your US Mail carrier.

Once your repair has been promptly made, you will receive a new box with another free prepaid mailer enclosed for your next repair.  All packing materials will be included as well.
